Employee Flu Vaccination

Our UOC Staff Flu Vaccination campaign will officially begin Monday September 17, 2012. Each floor has different people assigned for flu vaccination administration. Those people will post the times and dates they will be administering the employee vaccinations in their areas. They are as follows:
A Level and the first floor employees: Clark Monk and Janiel Wright
2nd Floor employees: Devin Reilly, Rebecca Kunze, and Derrick Peatross
3rd floor employees: Dori Cannard and Clark Monk
Everyone must have the vaccination. At this time, there are two acceptable reasons for declining the vaccination, they are: Allergic reaction or Religious Doctrine. Both reasons must be supported with documentation from a physician or clergy. IF YOU QUALIFY TO SIGN A LETTER OF DECLINATION AND DO NOT RECEIVE A FLU SHOT, YOU MUST FILL OUT THE DECLICNATION AT THE EMPLOYEE HEALTH CLINIC.
(Those who have experienced an allergic reaction in the past can go to the Employee health for their vaccination. A safe protocol has been established for those employees to receive their vaccinations.)
If you fall into one of the two mentioned categories and do not receive a flu vaccination, you will be required to wear a mask covering your entire nose and mouth until the Flu season has been officially closed in 2013. For further information, please refer to your manager.
Flu shots will be available at the UOC until October 15, 2012. After that date, you must go to the employee health office and receive your vaccination prior to November 1, 2012.
